To become an Authorized Agent for RDI Wireless, you must first obtain your RAD ID by visiting https://lifelinerad.org/rad/#/verifyRepresentative (link will open in new tab.  When done, please return to this page and complete this application.

IMPORTANT:  When you have completed this step, please COPY your RAD ID number and past it in the field below also forward the confirmation email you receive from RAD to rdiwireless22@gmail.com
